Work Permit Form

The State of Ohio requires Ohio teens between the ages of 14 and 17 to have a Minor Work Permit to hold a job during the school year. During the summer, only those under the age of 16 need a work permit. 

Please bring the completed forms (two pages) to the main office of Upper Arlington High School. The information provided will then be entered on the state's website, and then the work permit will be generated to give to your employer.
